All incoming Headstart, Pre-school, Pre-K, and Kindergartners need a physical, dental screening, vision screening, and a lead screening.
All incoming 3rd graders need a vision screening.
All incoming 7th graders need a Tdap and Meningitis vaccine or an exception.
All incoming 9th graders need a dental screening.
6th - 12th graders need to have a signed concussion form on file before participating in P. E.
7th – 12th graders who wish to participate in any physical sports practices or events must have a sports physical, concussion form and an emergency sports card to be allowed to participate. Both physicals and forms are to be renewed yearly.
All incoming 12th graders need two doses of the Meningitis vaccine or an exception.
All students need the required immunizations or exceptions on file before the first day of school.

Regular admission prices for home football games are $5.00 for adults and students. Other regular athletic events are $5.00 for adults and $4.00 for students in grades K-12. Senior citizens (65 and older) may pick up a complimentary pass, good for all regular conference athletic events. These are available at the Administration Office, 1103 N. Elm.
Balances from past due fees assigned to a student during the previous school year will be paid first before applying to this school year’s payments. Parents have the opportunity to pay these fees on-line by credit card, or at in-person registration if paying by cash or check. Once you set-up your account you may make payments for your family’s fees and lunch accounts. Call your student’s school secretary for directions if you need help getting started.
You will also be able to check your child’s lunch account balance anytime during the school year by going to the Infinite Campus parent portal site. Infinite Campus will also provide you with information on your child’s attendance report, grades, and teacher’s comments. The information that you will need to access the Infinite Campus parent portal site will be provided to you at registration.

Parents have the opportunity to pay for your child’s lunch account and student fees on-line with your credit card at TouchBase Online Payments. You can also pay at registration or any of our buildings.
Please provide proof of date of birth (for example, a birth or baptismal certificate) and immunization records for each child who will attend school in West Liberty for the first time. If your student is entering kindergarten or ninth grade and has had a dental and vision screening within the last year, please provide a document of proof.

Elm St. from 7:30 am - 4:00 pm Monday - Friday Having these documents filled out and ready to upload will help to speed up the registration process.

If left for too long without any activity your online registration will time out and incomplete sections will be lost. All complete sections should save so that you do not need to fill them out again. It would be very beneficial to have all of the above information readily available before beginning your registration.
Finally, remember to always keep the schools up-to-date with any life changes that affect emergency and/or home contacts. The Infinite Campus student/parent portal is made available to all parents/guardians and allows you to go online and make changes to your child’s emergency contact records. It also keeps you updated as to your child’s academic progress and attendance records. Call the school if you have any questions about using this system.
