Children's COVID Vaccine Clinic (ages 5-11)

Wester Drug has partnered with West Liberty School District in order to provide a Pfizer Covid-19 Vaccine Clinic for children between the ages of 5 and 11. The first dose would be administered Thursday, November 18 from 3:30 - 6:00 PM, and the second dose on Thursday, December 9th starting at 3:30 pm. Both clinics will take place at West Liberty Elementary School Gym, 806 N. Miller Street.  We ask that attendees enter through door number 2, the north main entrance facing Miller Street (there will be a sign showing you where to enter).After receiving their shot children will need to stay for a 15 minute observation period before leaving.

We ask that anyone interested please fill out the sign-up form linked below.  While signing up is preferred walk-ins will be welcome at the clinic as well.

Attendees will need to fill out the COVID Vaccine Consent form (linked below) to bring to the clinic on Thursday, November 18th. A parent/guardian will need to complete and sign the consent form since the children are under the age of 18. If you are unable to print the form using the link provided, you may pick up a copy in the main office of the Early Learning Center, Elementary School, Middle School, or in the District Office. If you would like your child to receive the vaccine but these times don't work you can reach out to Wester Drug to arrange a different time at one of their offices in Wilton or Muscatine.
